This day will appeal to pharmacists who have an interest in the management of people with cardiovascular disease and are keen to optimise the care provided. The masterclass will address the challenges of managing cardiovascular disease in the new NHS, in particular patients with atrial fibrillation and ischaemic heart disease. It will focus on recently published NICE guidance with particular reference to drug therapy and their role in current management guidelines, emphasising the importance of individualisation of therapy, and medicines optimisation. This masterclass is a must for band 7 pharmacists or those with an interest in the field.

Medicines optimisation in domiciliary settings – bridging the gap between primary and secondary care

Older people living in domiciliary and care home settings may often be frail and vulnerable and recent studies have shown that at least two-thirds of nursing home residents have been exposed to one or more medication errors, some of which may have serious, potentially fatal consequences. Improved medicines optimisation will promote the safe and effective use of medicines as well as increasing patient involvement in their healthcare.

Multi-specialty care for a patient with a diabetic foot

This day covers the journey of a complex patient with co-morbidities from clinic to hospital discharge via an in-patient stay. It will discuss medicines choice and optimisation, together with pain, infection and diabetic management. It will also cover how the patient is presented for surgery, including his or her pre, peri- and post-operative management, and the potential problems that may be encountered along the way. It is a must-attend event for pharmacists working in a generalist role.

The Critical Care – Advanced Practitioner Meeting is an essential day for every specialist pharmacist working in critical care. This year’s subject matter will as always be diverse and there should be something for everyone. The day will start with a review of the latest NICE Guidelines on intravenous fluid therapy and the implications for critical care. This will be followed with an update on the management of sleep disturbance, an overview of the impact of critical illness on the pk/pd characteristics of antibiotics and an audit of UK antibiotic administration practice. The day will be wrapped up with an introduction to critical care research. There will be plenty of opportunity to network with colleagues, share ideas for audit and research and find out about latest advances in the field.
